User and role audit

As a system owner, I need to audit all users and roles at least annually.

When auditing roles, I need to review module/feature granted to the role, and level of constraint (read, write, delete, etc).
When auditing users, I need to review associated roles, status and when last logged on. I user is identified by internal user ID and when applicable; AD identifier.
